Program transforms “illness to wellness”

Chronic diseases such as diabetes, heart disease or emphysema have a huge impact on individuals and their families but a Stanthorpe Hospital health education program aims to help people overcome these problems.
Chronic disease self management clinical nurse Helen Ferrier said she was holding a chronic disease self management program called ‘Illness to Wellness’ at the Stanthorpe Community Health Centre.
‘Illness to Wellnes’” is for people with a chronic disease who want to better manage the challenges of living day to day with their illness.
“The program recognises that to self manage and improve quality of life, a range of information, knowledge, skills and services need to be in place.
“The program is very comprehensive and seeks to be holistic in nature by looking at social, physical, mental, emotional, nutritional and environmental health elements.
“We have six sessions held weekly from 1pm to 3pm and our participants learn about a range of skills including goal setting, self management principles, physical activity, sleep health, nutrition, complementary health options and medication management,” she said.
Ms Ferrier said the program provided:
•    a small group setting with others who are also affected – with sharing of ideas and motivation;
•    a safe, comfortable, informal and relaxed learning environment;
•    range of information;
•    self management skills;
•    relaxation techniques;
•    goal setting and auctioning; and
•    skilled group facilitator and guest speakers in nutrition and physical activity
“There is no cost, however, an optional book or CD can be purchased for $15.
“Other health professionals also participate in the program or you may be referred to them for specialist health management — dietitian, social worker, physiotherapist or occupational therapist.
